У средневековых схоластов (богословов и юристов) казуистика представляла собой особый диалектический приём, при помощи которого какой-либо религиозный, моральный или юридический вопрос разбивается на бесчисленное множество мелких деталей и случаев, и, вместо решения вопроса в принципе, стараются войти в тончайший и исчерпывающий анализ всех возможных, и мысленно представимых, случаев. В диалектической казуистике особенно отличались иезуиты. С тех пор в теологии под казуистикой стали понимать и учение о степени греха применительно к различным обстоятельствам. Казуистика стала (особенно в католицизме) теорией «казусов совести», регулирующей конфликты между различными нравственными обязательствами человека, когда необходимо определить приоритет одного из них перед другим в конкретных сложных обстоятельствах.
В юриспруденции под казуистикой понимают анализ данного дела, случая (казуса) и выведение из него путём логической и юридической интерпретации некоторых общих принципов, дополняющих недостающие в законодательстве нормы. В судебной практике большинства стран нередко приходится прибегать к казуальному творчеству из-за выдвигаемых усложняющейся жизнью новых вопросов, ещё не отражённых в законе.
В медицине под казуистикой понимается «отдельное наблюдение (повреждения или заболевания), представляющее научный и практический интерес ввиду его редкости и необычности».

Самое интересное, невероятное, порой пугающее и требующее к себе повышенного внимания, случается вне всяких планов. Поэтому, скорее всего, мы не ошиблись, когда решили обратиться к такой благодатной специальности как неотложная хирургия, чтобы раскрыть несколько интересных случаев: чтобы было, что вспомнить, есть о чем напомнить и будет то, о чем нужно помнить. Нашим собеседником в этом вопросе стал заведующий кафедрой общей хирургии Казанского государственного медицинского университета, доктор медицинских наук, профессор Сергей Васильевич Доброквашин. Именно из его хирургической практики, с его слов мы узнали, что…
Дело было около 15 лет назад.
По «неотложке» поступил молодой парень, только отслуживший в армии, с острым флебонозным аппендицитом. «Мы его прооперировали, все проходило без всяких осложнений, технически все было сделано правильно, но неожиданно для нас через три дня начались проблемы», — рассказывает Сергей Васильевич. Череда осложнений началась с нагноения брюшной стенки, за ним последовал абсцесс брюшной полости, затем — перфоративные острые язвы тонкой кишки, острые язвы желудка и двенадцатиперстной кишки с кровотечением, дуоденальный свищ… «После каждого осложнения пациент оперировался. Было проведено более 10-ти хирургических вмешательств, но в чем дело, почему возникают осложнения, мы понять не могли. Занесение какой-либо инфекции было исключено».
Ситуация усугублялась психологическим фоном – парень воспитывался одной матерью, был единственной поддержкой и опорой в семье, необходимо было во что бы то ни стало сохранить жизнь и здоровье, не допустить инвалидизации.
«Наконец, состояние стало улучшаться, молодой человек выписался. Но ряд необъяснимых осложнений, возникших после обычной, несложной операции по удалению аппендицита, вызывали у нас беспокойство. И вдруг, на выписке, в ходе разговора с пациентом выясняется, что парень во время службы в армии получил однократную дозу облучения, то есть иммунитет временно был «угнетен», снижен. Лучевой болезни не было, но подобные нарушения возникли именно из-за снижения иммунитета. Пациент был направлен к иммунологам. А через полгода он опять поступает к нам со спаечной кишечной непроходимостью. Мы его оперируем, все проходит гладко, без всяких осложнений и дополнительных вмешательств», — говорит Сергей Васильевич. И добавляет: «Больше таких случаев не было в моей практике. Да и не дай Бог».
Профессор вспоминает еще одну ситуацию, связанную уже с другим заболеванием – холециститом. Случай произошел сравнительно недавно – пару лет назад.
«Одной из клинических баз кафедры является Госпиталь ветеранов войн, и соответственно, в нашем стационаре лечатся ветераны, пожилые люди. Пациентка поступила к нам с заболеванием сосудов нижних конечностей. Проводилось лечение, и примерно через неделю заболел живот. Пациентке было 89 лет, без двух месяцев 90 лет, поэтому мы стали проводить консервативное лечение, но оно оказалось малоэффективным. У пациентке диагностирован калькулезный холецистит, холедохоледиаз, механическая желтуха со всеми вытекающими последствиями. Мы ее прооперировали, убрали из желчного протока 36 камней. Через два месяца здоровая пациентка успешно отметила свое 90-летие», — говорит Сергей Васильевич.
Про молодежь и Кока-колу
Двое молодых людей и девушка чуть старше 20 лет, в компании отмечали надвигающийся День республики, поскольку дело было 28 августа 2006 года. Когда праздничная фантазия была полностью исчерпана, кому-то в голову пришла мысль засунуть девушке в прямую кишку бутылку Кока-колы. Когда девушка пришла в себя после алкогольного опьянения, почувствовав сильные боли, вызвала скорую помощь. Мы сделали снимок и получили ту картину, которая кратко и была описана. В операционной мы дали пациентке наркоз с миорелаксантами для расслабления мускулатуры передней брюшной стенки. Положили в гинекологическое кресло, пропальпировав инородное тело, осторожно, через переднюю брюшную стенку аккуратно продвинули донышко бутылки к анусу и без разрезов и без каких-либо дополнительных вмешательств, извлекли инородное тело.
Девушка, придя в себя после наркоза, сказала нам «Чао!» и ушла.
«Буйные» 90-е, и инородные тела
Картина была примерно той же, но ситуация иная – бутылку из-под жигулевского пива ввели во влагалище девушке. Бутылка, разорвав задний свод, попала в брюшную полость. Потребовалась лапаротомия. Случай был сложный, но состояние пациентки в послеоперационном периоде было достаточно хорошим. Выздоровление наступило быстро.
Зубы съел
Ситуация произошла с пожилым пациентом, который в состоянии опьянения не заметил, как проглотил зубной протез нижней челюсти. Протез был поставлен не по современным стандартам, а по-старинке, «на крючочках». Была повреждена ротоглотка именно этими «крючочками». Потребовалось эндоскопическое вмешательство, протез извлекли, пациент выздоровел.
Лучше не сталкиваться
Все эти «интересные» случаи в основном происходили в отделении неотложной хирургии, когда оно было развернуто в Госпитале ветеранов войн. Сегодня, когда мы занимаемся плановой хирургией, сложные случаи больше всего связаны с количеством сопутствующих заболеваний у пациентов, возрастными ограничениями для оперативных вмешательств, взаимосвязью с другими специалистами и так далее. У пожилого контингента свои, особые ситуации. О них поговорим в другой раз.

Казуистика в медицине
Молодая женщина 25 лет, медицинская сестра, беременность 24 недели.
Неожиданно ни для кого в левой почке обнаружено объемное образование очень больших размеров, гетерогенной эхоструктуры с акустической тенью. Было высказано предположение о злокачественной забрюшинной тератоме с прорастанием в почку.
На снимках левая почка и опухоль.
На снимках почечная артерия и единичные локусы кровотока по периферии опухоли.
На снимке левое забрюшинное пространство с опухолью и единичными забрюшинными лу, расцененными как мтс опухоли.
Пациентке проведено МРТ, осмотрена врачами онкологами в Москве. Биопсию опухоли не провели. Беременность решено было пролонгировать, опухоль наблюдать.
Беременность пациентка доносила, оперативное родоразрешение путем кесарева сечения. После родов на прием к онкологам не поехала.
После родов визуализация опухоли стала более отчетливой.
На снимках почка с опухолью.
В левом забрюшинном пространстве гетерогенные опухолевые массы со скудным кровотоком.
От операции отказалась.
